Integrated care systems

Connecting care settings across integrated care to drive better patient outcomes

Bleepa connects care settings and streamlines patient pathways to drive better, safer clinical decisions that improve outcomes for patients. By linking different clinical systems together into a seamless view of the patient, which is easy to use, we enable faster decisions and speed up patient care.

We lift data out of different and siloed healthcare IT systems and provide workflows to allow the clinician to see exactly where their patient is on the journey across primary, secondary, and community care and be able to easily ensure their exact needs are met.

Streamline patient pathways for quicker, more efficient care

Bleepa enables clinical teams to refer patients, share and review patient data and discuss cases collaboratively across care settings for better, faster delivery of patient care.

  •  Safer referral and discharge with all the relevant patient information – medical imaging, test results, treatment plans, clinical photos and more
  • Save time and reduce unnecessary referrals with greater collaboration across clinical areas
  • Connect care settings and bring clinical teams together in a patient-centric clinical chat for advice and guidance around patient management
  • Support virtual wards by retaining oversight and accountability of care at home

Prioritise patients against
diagnosis targets

Bleepa provides integrated care leaders with visibility of the status of patients and where they are in their care journey at any stage.

Ideal for cancer alliance, imaging and pathology networks, Bleepa’s simple, dashboard view, in the advanced stages of development, provides clear monitoring against faster diagnosis and other national targets to prioritise patients and speed up care.

The dashboard enables users to easily:

  • See patient status in a simple view
  • Receive breach alerts
  • Deep dive into case notes and test results
  • Chat directly with colleagues

Reducing diagnostic wait times by up to 70%

Initial data from the Bleepa symptom-based community diagnostic centre pathway is showing encouraging results to date, with an approximate 70% saving in patient wait times compared to the NHS 18-week referral to treatment (RTT) target, without the need for additional clinical staff. Evidence also shows that in 89% of cases there was a reduced need for outpatient appointments.

Time and cost efficiencies with Bleepa

Economic benefits identified in an independent evaluation of Bleepa estimates that the collected time savings from faster referrals, efficient clinical messaging and reduced length of stay for patients from the Northern Care Alliance’s use of Bleepa could unlock:

  • £819,000 over the five years to the 2027/28 financial year for the Trust
  • £7.7 million across the whole Greater Manchester integrated care board footprint over the same period.

Safer, accelerated patient discharge

With Bleepa, a patient can be discharged with the necessary clinical data, presented in a simple format, to give care providers confidence that everyone involved has the right information to provide the best step-down care.

Bleepa supports assessment of patient needs and is fully auditable, making it easy to quickly show that safe, effective care has been provided.

Whether the patient is discharged to another clinical setting such as a care home, or placed in a virtual ward for ongoing treatment and monitoring at home, Bleepa can ensure that the care team has the information they need for a holistic view of the patient’s needs.

Liberate clinicians with powerful communication tools

Equip staff with powerful communication tools that can seamlessly connect colleagues working within and between care settings to optimise their time and support remote working without disruption.

Communication can happen asynchronously or in real time through Bleepa. Colleagues can securely message each other on their own devices, meaning that team members can respond as soon as they are free and can fit consultations around their existing worklists.

On-call rotas can be shared across care settings and support local colleagues to work on-call shifts remotely by facilitating communication with on-site teams.

Facilitating virtual wards

With clinical teams able to collaborate around a patient wherever they are, with the key clinical information available to provide a holistic view of a patient’s care, Bleepa can help provide patient management and multi-disciplinary communication for virtual wards.

  • Retain oversight and accountability of care at home
  • Share a common patient view across care providers
  • Communicate across care settings asynchronously and directly
  • Access clinical support teams quickly for advice and guidance

By facilitating greater collaboration carers, nurses and community care teams can use Bleepa’s communication channels to quickly escalate to secondary care teams if a patient deteriorates, avoid hospital admissions where possible and reduce the rate of patients re-accessing services via A&E. They can utilise on-call teams and access the right expertise out of hours.

Facilitating cross-site communication

Bleepa can help new community diagnostic centres to deliver their potential by ensuring that these sites are properly connected to primary and secondary care settings and have the right digital infrastructure to present diagnostic information to these settings whilst facilitating cross-site communication.

Working with Queen Victoria Hospital (QVH), Bleepa has delivered the first live and operational end-to-end symptom-based pathway in the community diagnostic centre space. The pathway connects primary care providers, the new CDC and a multidisciplinary team of specialist consultants at QVH around a single patient journey for breathlessness.
